R44 , much like SR22, is a form that you must file with the state of Florida after particular convictions, typically after DUI convictions.
FR44 has highter limits than the SR22.
Drivers convicted of a DUI in Florida after February, 2008 are required to up their Liability limits.
The increase auto insurance liability limits are required for 3 years and are 100/300/50.
- $100,000 per person for bodily injury or death
- $300,000 total for any one accident for bodily injury or death
- $50,000 of property damage liability
Currently the reinstatement fee are:
- $150 – 1st offense
- $250- 2nd offense
- $500 – 3rd offense
Your chosen insurance company is responsible to file the FR44 form with DMV.
If the FR44 is not received by the DMV when your new auto insurance policy is issued or or policy is renewed, the DMV can suspend your privilege to drive and register motor vehicle.

Please note the following when completing an FR-44:
- All FR-44’s must be faxed to the Florida Bureau of Financial Responsibility at (850)-617-5216.
- All FR-44’s must be typed. Hand written forms will be rejected by the state.
- All fields must be completed on the filing or it will be rejected by the state.
- A copy of the FR-44 must be faxed to You Insurance Company.
- A copy of the FR-44 should be provided to the Insured
Please be aware that policyholders cannot have:
- a Florida FR-44 on a policy in a state other than Florida, or
- more than one FR-44 on one driver or,
- a Florida SR-22 and Florida FR-44 on one driver.
